What is Care Coordination?

Care coordination means different things to different people. That often leads to confustion as to who might need care coordination, or what services are covered under the unbrella of care coordination.

At Graceful Journey, care coordination means that we are here to help clients and families make informed care plans and care decisions with compassion, grace, and dignity. It is our goal that clients and families are able to navigate health care systems and transitions through life cycle stages with appropriate resources and support.

From helping you find community resources, providing patient/family education and support, to helping you create a long term care plan, we will be there every step of the way.

Our best found definition of care coordination is that“Care coordination is the deliberate organization of patient care activities between two or more participants (including the patient) involved in a patient’s care to facilitate the appropriate delivery of health care services.
